29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    a)  On or about April 17, 2014 workers involved with roofing construction at 210 Church Street, Saratoga Springs, NY 12866 were not wearing safety glasses and were potentially exposed to eye injuries from falling debris, while using hand tools, and while removing the roofing.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a)  On or about April 17, 2014 an employee was working on a ladder jack scaffold at approximately 15 feet above ground level at 210 Church Street, Saratoga Springs, NY was not protected by a personal fall arrest system.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a)  On or about April 17, 2014 at 210 Church Street Saratoga Springs, NY 12866 workers were performing roofing work on a steep roof of a slope of approximately 12 vertical on 12 horizontal without fall protection by a guardrail system, safety net or personal fall arrest system and accordingly exposed to a possible fall of approximately 20 feet.
