29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) Job Site, 91 Hillside Ave, New Rochelle, NY: Two employees  removing snow from the metal corrugated sheets of the roof, in order to set up plywood and build a new roof, were not protected from falling through six (6) skylights , on or about 4/16/2014.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:        a) Job Site, 91 Hillside Ave, New Rochelle, NY:  Two employees removing snow from metal corrugated sheets of the roof approximately 15 feet above the ground, were not protected from falling; on or about 4/16/2014.

29 CFR 1926.1053(b)(1): Where portable ladder(s) were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a) Job site, 91 Hillside Ave, New Rochelle:  The aluminum 36' high "Werner" extension ladder used by the employees to gain access/egress from the roof was not secured nor did extend three(3) feet above the ground; on or about 4/16/14.
