29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: a) Mohawk View Water Treatment Plant Screen and Grit Building - On or around March 17, for the employees that entered the Screen and Grit Influent Channels to conduct repairs and gate replacement without training on potential hazards. Hazards include biological hazards (pathogens such as bacteria, viruses, protozoa, parasitic worms, fungi), slip and trip hazards, limited access and egress. b) Mohawk View Water Treatment Plant Grit Tank #1 - On or around March 17, for the employees that entered the Grit Tank #1 to conduct repairs and gate replacement without training on potential hazards. Hazards include biological hazards (pathogens such as bacteria, viruses, protozoa, parasitic worms, fungi), slip and trip hazards, limited access and egress. c) Mohawk View Water Treatment Plant Grit Tank #2 - On or around March 17, for the employees that entered the Grit Tank #2 to conduct repairs and gate replacement without training on potential hazards.
