29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.   a. At the worksite where employees were exposed to chemicals such as, but not limited to Mapei Ultraflex 2 mortar containing Portland Cement, employees were not trained on the hazards of the chemicals, labels, and safety data sheets, thereby being exposed to contact hazards.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a. In the hallway of area E where employees plugged in an extension cord, the ground prong of the extension cord was missing, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. In the first floor of area E where employees were using an extension cord to provide power to a tile saw, the strain relief of the plug was pulled away from the outer insulation jacket of the extension cord, thereby exposing employees to an electrical contact hazard.
