Employee was observed operating a Makita grinder with the guard missing over the cutting wheel exposing the employee to eye and or face injuries.       Condition occurred on or about 04/17/14.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 392 Amboy Avenue, Woodbridge, NJ 07095. Two employees were observed accessing the roof of the Reo Diner using a portable aluminum extension ladder that did not extend three feet above the landing.    Condition occurred on or about 04/17/14.

Two employees were observed working on and from fiberglass "A-Frame"  ladders in the closed position.     Condition occurred on or about 04/17/14.
