Employee was observed operating a Makita grinder with the guard missing over the cutting wheel exposing the employee to eye and or face injuries. Condition occurred on or about 04/17/14.

Two employees were observed accessing the roof of the Reo Diner using a portable aluminum extension ladder that did not extend three feet above the landing. Condition occurred on or about 04/17/14.
