On or about April 17, 2014, and at times prior thereto, the competent person did not ensure that used filters on negative pressure enclosure system were replaced and disposed after use. This condition exposed employees to health hazards.

29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures. on or about April 17, 2014, and at times prior thereto, employees were using a Shurflo pump that required 115 Volts and connected with two electrical wires that were not guarded against accidental contact. This condition exposed employees to electrical hazards.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about April 17, 2014, and at times prior thereto, employees were using an extension cord with missing ground pin to connect the Shurflo water pump. This condition provided a path to ground that was not permanent and continuous, exposing employees to electrical hazards.
