You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.   Located at Nissen Chemitec America in London, Ohio:  a.       On the 2014 OSHA 300 Log the employer did not provide all information required in Column F such as the actual injury, the cause of injury and the body part affected.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a.  At the workplace, the employer did not certify that periodic inspections of the energy control procedures were performed to ensure that the procedure and the requirements of the standard are being followed.
