29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets: On or about April 10, 2014, at 8558 Jefferson Highway, Baton Rouge, LA, two employees were fabricating and installing sheet metal roofing on a commercial building at a height of approximately 10 feet without a means of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about 04/10/2014, at 8558 Jefferson Highway, Baton Rouge, LA, employees were accessing the roof of a commercial building with a ladder that was not 3 feet above the landing, and not secured.
