29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Location: 309 Meadowlark Lane, St. Charles, MO 63301 The employer failed to have a competent person inspect the trench prior to the start of the sewer repair job. A worker was authorized to enter an excavation without a protective system in the excavation that measured approximately 6.5 feet in depth, 3 feet in width, and 20 feet in length.
