29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   On October 2, 2014, Sean Cole Construction, Inc. was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:           a.    Citation 1 Item 1a & b, and      b.    Citation 2 Item 1, and      c.    Citation 3 Item 1.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees used an extension ladder to access a roof approximately 18 feet from the ground and the employer did not ensure that the ladder's side rails extended at least three feet above the roof's eave.

29 CFR 1926.1060(a):   The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:   Employees used an extension ladder to access a roof approximately 18 feet from the ground and the employer did not ensure that the employees were trained to recognize and avoid unsafe ladder usage.

29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Employees were exposed to a fall hazard of approximately 18 feet to the ground below while installing tar paper and shingles and a suitable fall protection system was not utilized.   SEAN COLE CONSTRUCTION, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314849803, citation number 2, item number 1 and was affirmed as a final order on Apil 20, 2011, with respect to a workplace located at 103 Adloff, Springfield, IL 62711.  SEAN COLE CONSTRUCTION, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 310804034, citation number 1, item number 1a and was affirmed as a final order on December 19, 2008, with respect to a workplace located at 512 Wake Court, Chatham, IL 62629.
