29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a)  Paul Fox & Sons Excavating Inc. on or about April 18, 2014 at the address of 222 McDonough St. Sandusky, OH: The employer failed to ensure a safe means of egress, such as a ladder was provided to employees installing sanitary pipe in a 7 ft. deep trench.

(a)  Paul Fox & Sons Excavating Inc. on or about April 18, 2014 at the address of 222 McDonough St. Sandusky, OH: The employer failed to ensure employees were adequately protected from cave-ins while installing a residential sanitary line in a 7 ft deep unprotected trench.
