29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a. On or about April 18, 2014- at a jobsite located at 7001 S. Stony Island Avenue, Chicago, IL.- employees were performing masonry activities from a tubular welded framed scaffold that was not equipped with an access ladder their work zone, thereby exposing the employees to fall hazards of approximately ten (10) feet, three (3) inches.

Stony Island Avenue, Chicago, IL.- employees were performing masonry activities from a tubular welded framed scaffold that was not equipped with guardrails or endrails, thereby exposing the employees to fall hazards of approximately ten (10) feet, three (3) inches.
