29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews.         On or about January 8, 2016, at the above addressed jobsite, masonry workers engaged in bricklaying activities were:  a. Working from the second level of a tubular welded scaffold and the scaffold was not equipped with mid rails to protect workers from falling to lower levels.   b. Working from the second level of a tubular welded scaffold with outriggers on both ends of the platform while bricklaying in front, however the platform behind the worker was not fully planked to withstand at least 200 pounds in order to protect the worker from falling to lower levels.  Choice Masonry, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.451(g)(1) which was contained in OSHA inspection number 968737, citation number 1, item number 2a and was affirmed as a final order on June 17, 2014, with respect to a workplace located at 7001 S. Stony Island Chicago, Il.
