29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On 16 April 2014, six workers were performing framing activities at a residence under construction in which overhead work was ongoing, and materials were being handed up, while not wearing head protection, exposing the workers to the hazard of being struck by falling debris, tools, or materials.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On 16 April 2014, a framer was using a pneumatic nail gun without the use of eye protection to perform framing work on a residence under construction, exposing the employee to the hazard of being struck by flying objects.
