29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) Jobsite, 5800 Wellesley Street, Pittsburgh, PA: On or about 04/17/14 the employer did not retrain employees who were working in the aerial lift without the proper fall protection indicating the employees did not retain the knowledge and requisite proficiency to protect them from fall hazards.
