29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold shall be protected by a guardrail system (with minimum 200 pound toprail capacity) installed within 9 1/2 inches (24.1 cm) of and along at least one side of the walkway: (a) Jobsite, 362 S. Highland Avenue, Pittsburgh, PA: On or about 04/17/14 the walkway erected between the two scaffold towers did not have guardrails installed to protect the employees traveling between them from fall hazards of approximately 25 feet to the ground below.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) Jobsite, 362 S. Highland Avenue, Pittsburgh, PA: On or about 04/17/14 the employer failed to retrain an employee working on the scaffold when their work performance indicated that the employee had not retained the requisite proficiency to work safely. An employee was working on a scaffold approximately 25 feet above the ground with no guardrails in place indicating the employee had not retained the knowledge and skill necessary to recognize hazards and what procedures should be used to protect them when working on a scaffold.
