29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (Construction Reference 1926.103)   a) Location - 75 Leonard Street, Belmont, MA:   On or about April 18, 2014, for employees provided and required by the employer to wear respiratory protective devices the employer did not provide or ensure employee's were medically able to wear the devices.  Employees were exposed to air contaminants such as, but not limited to: respirable quartz silica, nuisance dust.

29 CFR 1910.134(k)(1): The employer did not provide respirator training that would ensure each employee could demonstrate knowledge of items in section (i)-(vii): (Construction reference 1926.103)  a) Location - 75 Leonard St., Belmont, MA:  On or about April 18, 2014, employees required by the employer to wear N95 respirators during demolition work were not provided the required training.      The Cam's Demolition & Disposal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.134(k)(1)), which was contained in OSHA inspection number 315860411, citation number 1, item number 1 and was affirmed as a final order on 03/28/2012, with respect to a workplace located at 30 Porter Road, Littleton, MA.
