29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  (a)  Phase Hanger:  On or about April 16, 2014 and times prior thereto, a flexible cord did not have proper strain relief to prevent pull.  This exposed employees to potential shock hazard if the connectors touched each other from pull on the cable.
