29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by nip points and rotating parts: On or about April 16, 2014, and times prior to, at 1201 North 8th Street in Rogers, Arkansas, (a) the unused portion of the cutting blade was not guarded on the Ellis horizontal band saw, #1038, located in Department 10 Gun area. This condition exposed employees to contact with the cutting blade. (b) the unused portion of the cutting blade was not guarded on the Kalamazoo horizontal band saw, #1020, located in Department 07. This condition exposed employees to contact with the cutting blade. (c) the CBN wheels and spindle ends on the Darex drill sharpener, #40851, located on the south wall in the Die Repair area were not guarded. This condition exposed employees to contact with rotating parts. (d) the cutting wheel and spindle end on the left side of the Black & Decker 6" heavy duty bench grinder located on the east wall in the Die Repair area were not guarded. This condition exposed employees to contact with rotating parts. (e) the 40" chuck on the Crown-6 lathe, #3008, located in Department 03 was not guarded. This condition exposed employees to contact with rotating parts. (f) the Makino machine #3036, located in Department 10 Gun area, was not guarded to prevent employees from reaching over the front door cover. This condition exposed employees to contact with rotating parts. (g) the Makino machine #3037, located in Department 10 Gun area, was not guarded to prevent employees from reaching over the front door cover. This condition exposed employees to contact with rotating parts. (h) the cutting wheel on the Cincinnati horizontal mill located in Department 10, was not guarded. This condition exposed employees to contact with rotating parts. (i) the upper exposed sanding belt and rotating pulley-drum on the Haas 1" sander/grinder, Model 351.226110, located in Department 10 Gun area, were not guarded. This condition exposed employees to contact with in-running nip points. (j) the Un-Winder machines located in the press area, used to unspool metal, were not guarded.
