(a)    Left Side Scaffold - On or about April 17, 2014 employees were exposed head injuries while an employee was working on a scaffold 81 inches above ground.  Employees on ground below scaffold were not wearing hardhats.      (b)   Right Side Scaffold - On or about April 17, 2014 employees were exposed head injuries while an employee was working on a scaffold 17 feet 3 inches above ground.  Employees on ground below scaffold were not wearing hardhats.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows:     (a) Right Side Scaffold - On or about April 17, 2014 employees were exposed to a fall hazard due to a scaffold not being fully decked or planked. Three 12 inch boards were installed on the scaffold resulting in a 24 inch gap where employees could fall 13 feet to the ground.

29 CFR 1926.451(b)(5)(ii): A platform greater than 10 feet in length was extended over its support more than 18 inches (46 cm), and was not designed or installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.   (a)  Right Side Scaffold - On or about April 17, 2014 employees were exposed a fall hazard due to the platform boards being extending over the supports greater than 18 inches. The right edge of the platform was 6 feet over the support.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections were not secured.  (a)  Left Side and Right Side Scaffolds - On or about April 17, 2014 employees were exposed to a fall hazard while working on a scaffold that was missing cross-braces on the front.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces shall not be used as a means of access.     (a) Right Side Scaffold - On or about April 17, 2014 three employees were exposed to a fall hazard while working on a scaffold that had no ladder installed. Employee climbed scaffold frame 17 feet 3 inches above the ground to access the scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. The training shall include the following topics, as applicable:   (a)  Construction Site - On or about April 17, 2014 employees were not provided training on how to recognize, prevent and eliminate fall hazards when working from scaffolds.
