The employer did not provide audiometric testing for an employee exposed over the action level of 82.1 dBA, on or about April 25, 2014.   b)  Custom Particle Reduction - An Operator working at work station #2 was exposed to continuous noise at 167.7% of the permissible exposure level of 90 dBA or 11 hour time weighted average of 93.7 dBA.  This exposure was observed over a 660 minute sampling period.  The employer did not provide audiometric testing for an employee exposed over the action level of 82.7 dBA, on or about April 25, 2014.  c)  Custom Particle Reduction - An Operator working at work station #4 was exposed to continuous noise at 91.84% of the permissible exposure level of 90 dBA or 10 hour time weighted average of 89.3 dBA.  This exposure was observed over a 615 minute sampling period.  The employer did not provide audiometric testing for an employee exposed over the action level of 83.4 dBA, on or about April 25, 2014.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

The purpose of audiometric testing and an explanation of the test procedures.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
