29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 4/15/2014, two employees from Artisun Solar, LLC were working on the roof of the Sun River Village Pool House located at the intersection of Broadridge Lane and Sequoia Drive, St Peters MO. The two employees were utilizing the aerial lift to hoist materials and access the roof. The employees were not wearing body belts with lanyards attached to the boom or basket while in the aerial lift.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 4/15/2014, two employees from Artisun Solar, LLC were working on the roof of the Sun River Village Pool House located at the intersection of Broadridge Lane and Sequoia Drive, St Peters MO. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.
