29 CFR 1910.119(j)(2):  The employer did not establish and implement written procedures to maintain the on-going mechanical integrity of process equipment.      The employer does not ensure written procedures are established and implemented to maintain the on-going mechanical integrity of the process equipment.      A.    The violation occurred in the HYCO#1, HYCO#6, and Formaldehyde units on or about April 17, 2014 and at times prior thereto where the employer failed to ensure written procedures were established and implemented for the testing and inspection of process equipment interlocks and controls identified as safe guards and/or safety systems.  Identified safe guards and/or safety systems include but not limited to:      a.  PAH 4602 (Pressure Alarm High)  b.  ISL 4501/02  (Current Switch Low)  c.  FSLL 4010 (Flow Switch Low Low)  d.  TAH (Temperature Alarm High)  e. TI-4017 (Temperature Indication)  f.  PSHH 4033/4076/4076A (Pressure Switch High High)  g.   LSLL 4204/4212 (Level Switch Low Low)  h.   TSHH 4412/4405 (Temperature Switch Low Low)  i.  FISL 4402 (Flow Indication Switch Low)  j.   PISLL 4023 (Pressure Indication Switch Low Low)  k. PDAL 103 (Pressure Differential Alarm Low and Shutdown)      B.    The violation occurred in the HYCO#1, HYCO#6, and Formaldehyde units on or about April 17, 2014 and at times prior thereto where the employer failed to ensure written procedures were established and implemented for the identification and selection of corrosion/thickness monitoring locations for testing and inspection of process equipment pressure vessels and piping.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure written procedures are established and implemented to maintain the on-going mechanical integrity of the process equipment.

29 CFR 1910.119(j)(4)(ii):  Inspections and testing procedures performed on process equipment to maintain its mechanical integrity, did not follow recognized and generally accepted good engineering practices.      The employer does not ensure inspections and tests performed to maintain the on-going mechanical integrity of process equipment followed recognized and generally accepted good engineering practices (RAGAGEP).      The violation occurred on or about April 17, 2014 and times prior thereto in the HYCO #1 & #6 Units where the employer failed to ensure inspections and testing procedures performed on process equipment piping was in accordance with RAGAGEP such as API 570.
