Hammer Time Construction LLC worksite located on Putnam Parkway, Ottawa, Ohio: On or about April 18, 2014, the employer did not ensure employees were protected from fall hazards while performing framing activities on the Southeast corner of the 400 Wing. Employees were exposed to a fall distance of approximately 10.00 feet while working on the top plate of the exterior wall.

Hammer Time Construction LLC worksite located on Putnam Parkway, Ottawa, Ohio: On or about April 18, 2014, the employer did not ensure employees were prevented from using the top step of the 8 foot fiberglass step ladder located on the Southeast corner of the 400 Wing. An employee was exposed to a fall distance of approximately 8.00 feet while working on the top of the stepladder. B. Hammer Time Construction LLC worksite located on Putnam Parkway, Ottawa, Ohio: On or about April 18, 2014, the employer did not ensure employees were prevented from using the top step of the 8 foot fiberglass step ladder located on the Southwest corner of the 400 Wing. An employee was exposed to a fall distance of approximately 8.00 feet while working on the top of the stepladder.
