29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in �1926.502 (see Appendix G to this subpart): 29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about April 21, 2014, and at times prior thereto, a self retracting life line was not taken out of service where it had damage to the wire rope.
