Section (5)(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to crushing hazards from automotive lifts:  Main Shop Area - On or about 4-22-14 - Employees used hydraulically driven mechanical lifts for vehicle repair and maintenance including but not limited to the following instances, employees were exposed to crushing, falling and strain hazards created by the automotive lifts due to component failures.  a)  Bay #3, Challenger Spirit 2 - Safety catches were damaged and the lift was not taken out of service.  The lifts are leaking hydraulic fluid and lift arms are bent and not properly aligned to evenly support the vehicles on them.  The employer did not ensure that the manufacturers operating instructions, safety information, safety tips and warning labels were displayed in a conspicuous location in the lift area as required by ANSI/ALI (American National Standard/Automotive Lift Institute) B153.1 - 1990 and ALI ALOIM (Automotive Lift Institute) 1994 Section 4.2.2.  b)  Bays 1,2 & 3 - The employer did not ensure that each lift operator followed the recommended inspection procedures of the lift manufacturer and at a minimum noted that following points on a daily basis as required by ALI ALOIM - 1994 Section 4.4.4.  c)  Bays 1,2 & 3 - The employer did not institute a preventive maintenance procedure to ensure the continued safe operation of each lift as required by ALI ALOIM - 1994 6.1.1.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Bay #3 - On or about 4-22-14 - Employees were exposed to rotating parts and flying chips from unguarded Safe-Turn model 4000, brake lathe exposing employees to lacerations and eye injuries.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a)  Bay #3 - On or about 4-22-14 - Employees were using Craftsman six inch bench grinder with tool rest not adjusted to within an eight inch to the abrasive wheel.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  a)  Bay #3 - On or about 4-22-14 - Employees using Craftsman six inch bench grinder were exposed to laceration hazards from missing tongue guard on the grinder.
