Employees were exposed to an entrapment hazard.

Employees were exposed to a tip-over hazard from operating it.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily: 1) Production Area - Employees are operating a Caterpillar forklift truck, model 50DSA and serial 8JC624 where the tires were worn and defected. The employer did not ensure employees were conducting daily inspections before allowing employees to operate the forklift truck. Employees were exposed to a struck-by or tip-over hazard.
