29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations and procedures including the hepatitis B vaccine and vaccination series and post-exposure evaluation and follow-up, including prophylaxis, are provided according to the recommendations of the U.S. Public Health Service current at the time these evaluations and procedures take place, except as specified by paragraph (f). a) Fairmont Care Center, Inc., Chicago, IL- The employer did not ensure that employees consenting to and being provided the hepatitis B vaccination series by the employer were provided post-vaccination serological testing for antibody to hepatitis B surface antigen (anti-HBs) within one to two months after the final dose of the vaccine is administered, in accordance with the current recommendations of the U.S. Public Health Service guidelines. b) Fairmont Care Center, Inc., Chicago, IL- The employer did not ensure that all medical evaluations and procedures including the hepatitis B vaccine and vaccination series and post-exposure evaluation and follow-up, including prophylaxis, were provided according to the recommendations of the U.S. Public Health Service to three employees following an exposure incident . In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
