29 CFR 1910.146(c)(5)(ii)(C): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but before an employee entered the space, the internal atmosphere was not tested with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants:    a/ Fuel Bio One/Elizabeth NJ Site: Yard Tank Area.  Bio Diesel Reproduction Tank Approx. 45 ft. High and 20 ft. in Diameter:     The employer did not ensure that employee(s) did not enter the tank where the oxygen content by volume, could be  below 19.5 percent and/or if other contaminants were present in that, means were not provided to adequately test the tank prior to entering.     Observed on/or about 04/22/2014.

29 CFR 1910.335(a)(1)(i):   Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:     a/ Fuel Bio One/Elizabeth NJ Site: Yard Tank Area.  Employees are not provided appropriate electrical protectors, such as but not limited to electrical rated gloves and face shields when performing live electrical work.       Where employees are required (on a limited basis) to perform live electrical testing and/or trouble-shooting on machinery/ equipment, protectors including but not limited to approved electrical gloves (ANSI rated 00 or 01 - minimum 500 volts) were not provided for each electrical worker.       Employee performs electrical maintenance/testing using a "Multi-Meter."  Electrical equipment rated 460 volt 3 phase: including but not limited to motors, pumps, mixers/chillers and boiler      Observed on/or about 04/22/2014.
