29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Tool Room - On or about April 22, 2014, the employer did not develop procedures for the control of hazardous energy on machinery and equipment, to include but not limited to CNC Milling Machines, Horizontal and Vertical Bandsaws, and Surface Grinders, on which employees perform servicing and maintenance, including set up.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees as required by paragraph (c)(7)(i)(A) through (c)(7)(i)(C):    (a)  Throughout Facility - On or about April 22, 2014, employees who perform servicing and maintenance on equipment and machinery with hazardous energy sources such as, but not limited to CNC Milling Machines, Horizontal and Vertical Bandsaws, and Surface Grinders, were not provided with training to the authorized level, in that they were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.     (b)  Throughout Facility - On or about April 22, 2014, operators of equipment and machinery with hazardous energy sources such as, but not limited to but not limited to CNC Milling Machines, Horizontal and Vertical Bandsaws, and Surface Grinders, were not provided with training to the affected level, in that they were not instructed in the purpose and use of the energy control procedure.     (c)  Throughout Facility - On or about April 22, 2014, all other employees whose work operations such as, but not limited to material handling and parts sorting, are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.
