29 CFR 1926.1053(a)(1)(ii): Non-self-supporting portable ladder(s) were not capable of supporting without failure, at least four times the maximum intended load:      a)  Jobsite in Altamont, NY front of building - On or about April 23, 2014, a Werner aluminum extension ladder (Type III, 200lb load capacity, Model D1116-2) was used to access a roof exposing employees to a fall hazard of approximately 10 feet to ground level. An employee carrying tools and supplies exceed the load capacity of the ladder.        b)  Jobsite in Altamont, NY rear of building - On or about April 23, 2014, a Werner aluminum extension ladder (Type III, 200lb load capacity, Model D1116-2) was used to access a roof exposing employees to a fall hazard of approximately 10 feet to ground level. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:   a)  Jobsite in Altamont, NY front of building - On or about April 23, 2014, an extension ladder used to access a roof was not extended at least 3 feet above the eave exposing employees to a fall hazard of approximately 10 feet to ground level.    b)  Jobsite in Altamont, NY rear of building - On or about April 23, 2014, an extension ladder used to access a roof was not extended at least 3 feet above the eave exposing employees to a fall hazard of approximately 10 feet to ground level.
