29 CFR 1926.501(b)(1): Employees on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:  a) Block #353.06 / Lot #1, Building #38, Great Beds Court, Perth Amboy, NJ 08861. The employer allowed his employees to work on and from the newly constructed second floor deck installing floor trusses with no fall protection systems in place exposing them to fall hazards.  Condition occurred on or about 04/19/14.

29 CFR 1926.501(b)(14): Employees working on, at, above, or near wall openings, where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  a) Block #353.06 / Lot #1, Building 38, Great Beds Court, Perth Amboy, NJ 08861. The employer allowed his employees to work at or near an unprotected door opening with the outside bottom edge being in excess of 6 feet above the floor below with no fall protection systems in place.     Condition occurred on or about 04/19/14.

The employer allowed his employees to access and exit the building using a Werner, 24 foot aluminum extension ladder with broken and damaged rungs exposing them to fall hazards.   Condition occurred on or about 04/19/14.
