29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:  a.  4580 Riverside Drive, Macon, GA (Jobsite) - Employees were exposed to struck-by hazard.  The employee is loading and unloading the steel countertops using the powered industrial truck, transporting countertops into the moving truck.  The steel countertops was not secured to the forks of the forklift, causing the countertops to be displaced and/or shift from side to side while operating the forklift, the countertops fell from the forks of the lift, causing injuries to the forklift operator, on or about April 21, 2014.
