29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. Cantelli Block and Brick, Inc. - Huron, Ohio: On or about April 22, 2014, the employer did not develop and implement an energy control program consisting of training, procedures, and audits, to protect employees from hazardous mechanical and pneumatic energy sources. Employees that cleaned the mixer and/or set up molds and cleaned the Besser block machine were exposed to amputation and caught-in injuries while servicing the machines.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Cantelli Block and Brick, Inc. - Huron, Ohio: On or about April 23, 2014, the employer did not assure that rotating parts and pinch points of the Besser block machine were adequately guarded to protect employees from amputation injury. Barriers did not prevent employees from accessing hazard areas located at the side and front of the machine during operation.

Cantelli Block and Brick, Inc. - Huron, Ohio: On or about April 23, 2014, the employer did not assure that sprockets of the chain conveyor, located in front of the Besser machine to move blocks from the machine, were guarded to protect employees from being injured by the nip points.
