29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of hazard of crushing injuries from equipment rollover: a) Lawn Area of Site- Employees operate Toro Z Master Professional 5000 Series Riding Mowers without using the mower's Rollover Protective Systems (ROPS) and seatbelts as specified in the Operator's manual for the mowers. On April 22, 2014, an employee suffered a fatal accident when his mower rolled over while being operated on a 30 to 45 degree slope. The mower did not have the ROPS deployed and the employee was not wearing a seatbelt. b) 24 Gate Entrance - An employee operated a Toro Z Master Professional 5000 Series Riding Mower, Model Number 74915, Serial Number 312000579, on a 30 to 45 degree slope on April 22, 2014 when the mower overturned and caused fatal injuries to the operator. The operator's manual for this mower states that it not be operated on slopes that exceed 15 degrees. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
