29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: The employer does not ensure that each employee, who works from a scaffold, is trained. This violation occurred on or about April 17, 2014, at a jobsite located at 1515 N. Town East Blvd., Suite 168, Mesquite, Texas; where at least one employee, who was working from a mobile scaffold, was not trained to recognize the hazards associated with the scaffold, and on the procedures to be followed to minimize his exposure to the hazards.

29 CFR 1904.39(a): Within eight (8) hours after the death of an employee from a work-related incident, the employer did not orally report the fatality by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that was nearest to the site of the incident, or by calling the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742): Within 8-hours after the death of an employee that occurred on or about April 20, 2014, as the result of a workplace incident that occurred at 1515 N. Town East Blvd., Suite 168, Mesquite, Texas, the employer did not report the incident to OSHA.
