29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: At this job site, employees performing roofing activities were not trained in the following: A. Employees were not trained in how to identify existing and predictable hazards in the surrounding work area. B. The employer had not designated and trained a person to be a competent person for identifying dangerous conditions such as open holes and the lack fall protection measures.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. At this job site, employees installing insulation onto the roof were exposed to roof openings/holes, thus exposing the workers to a 20 foot fall to the concrete foundation.
