29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets;   a) Jobsite: 211 South 17th Street, roof - Employees engaged in roofing operations on a low-slope roof (4 in 12) were exposed to fall hazards of approximately twenty-eight (28) feet without the use of fall protection.  Observed on or about 04/23/2014.

29 CFR 1926.1053(b)(1): Where the portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder;  a)Jobsite: 211 South 17th Street. The portable extension ladder used to access the roof did not extend at least three (3) feet above the upper working surface exposing employees to a fall hazard during access and/or egress.  Employees were installing rubber roofing membrane on the roof, approximately twenty-eight (28) feet above the ground. Observed on or about 04/23/2014.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall;   a) Jobsite: 211 South 17th Street - Employees were exposed to fall hazards of approximately twenty-eight (28) feet while carrying roofing items up to the roof on a portable ladder. The employees were not able to keep 3-points of contact on the ladder when climbing.  Observed on or about 04/23/2014.
