Observed on or about 04/23/2014.

29 CFR 1926.1053(b)(1): Where the portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder; a)Jobsite: 211 South 17th Street. The portable extension ladder used to access the roof did not extend at least three (3) feet above the upper working surface exposing employees to a fall hazard during access and/or egress. Employees were installing rubber roofing membrane on the roof, approximately twenty-eight (28) feet above the ground. Observed on or about 04/23/2014.

The employees were not able to keep 3-points of contact on the ladder when climbing. Observed on or about 04/23/2014.
