Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    Jobsite:    Employee exposed to falls while working in a man basket attached to a forklift (Lull) with no fall protection and missing guard rail from the attachment man basket.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high.  Jobsite:  Employees using a work platform attached to the Lull forklift (with missing front guard rail) while working on the third floor window exposed to falls greater than 6 feet.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:(a) inside building 4 on the left side of the driveway where employees had an extension ladder temporarily going from the garage level to the 1st floor with no guard rails to prevent employees from falling.    Jobsite:    Employees were exposed to falls for not installing guard rails around an unprotected side or edge while accessing and egress the to the 1st floor.
