29 CFR 1910.1048(h)(1)(i): All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation: a) Mixing Room - On or about 4/23/14, employees who work with Varcum 29353 (a liquid phenolic resin containing up to 1% formaldehyde) were not provided with chemical protective clothing to cover the lower portions of their body including their legs. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1048(i)(2): When employees' skin could have become splashed with solutions containing 1 percent or greater formaldehyde, the employer did not provide conveniently located quick drench showers and/or did not ensure that affected employees used these facilities immediately: a) Mixing Room - On or about 4/23/14, employees work with Varcum 29353 (a liquid phenolic resin containing up to 1% formaldehyde). Employees' skin may become splashed when dispensing the resin. A quick drench shower was not available in the area for employee use. ABATEMENT CERTIFICATION REQUIRED

Employees eyes may become splashed when dispensing the resin. An acceptable eyewash facility was not located within the immediate work area. ABATMENT CERTIFICATION REQUIRED
