29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about March 14, 2014, the employer did not ensure that employees working from a height of approximately 50-60 feet wore fall protection.  Employee(s) were engaged in cutting down a jack up rig for scrap metal.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  On or about March 14, 2014, employee(s) working from a rig platform approximately 50-60 feet high did not receive fall protection training as specified in 29 CFR 1926.503(a)(2)(i) through (viii).
