Walnut, Columbia, Missouri- Employees were exposed to a fire or explosion hazard wile performing construction activities. Two fuel tanks, one filled with gasoline and one filled with diesel were located outside the jobtrailer and an extinguisher was not conspicuously located in the event or a fire or explosion.

Worksite located at 1414 Grindstone Plaza Drive, Columbia, Missouri- Employees were exposed to a fire or explosion hazard in that 3- 55 gallon drums containing kerosene were located such that a vehicle or piece of machinery could come in to contact with the drums and create an explosion, fire, or spill hazard.
