29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).     The employer does not ensure that all working levels on scaffolding are fully planked or decked. This violation occurred on or about April 24, 2014, at a construction site when an employee was exposed to a fall hazard while attaching bricks on the second floor of the structure using a scaffold without the working platform being fully planked or decked between the front uprights and the guardrail supports.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels on all scaffolds are fully planked or decked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.  The employer does not install guardrail systems along all open sides of scaffold platforms.  This violation occurred on or about April 24, 2014, at a construction site when an employee was exposed to a 12 foot, 2 inch fall hazard while performing masonry work from the second level of a scaffold without guardrails on the sides and ends.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guardrails are installed on all open sides and ends.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     The employer does not provide access ladders or other means for accessing scaffold platforms.  This violation occurred on or about April 24, 2014, at a construction site when an employee was exposed to a 12 foot, 2 inch fall hazard while doing masonry work from the second level of a scaffold without adequate access.        Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.
