29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: Establishment: In the following areas there were floor hole into which employees could accidentally walk: a) Wastewater Treatment Room - there are a 12-inch by 12-inch hole as deep as 4 inches in the center of the room. b) Wastewater Treatment Room - there was a gap in the diamond plating on the floor that measured 10 inches by 2.25 inches. c) Polishing Room ICF Plating Line - there was an 11-inch by 6-inch hole in the grating near Tank #18 that contained nitric acid. d) Polishing Room ICF Plating Line - there was an 11-inch by 6-inch hole in an adjacent section of grating also near Tank #18.
