29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred: (a) National Zoological Park - A Gardener received work related injuries while handling mulch on April 3, 2014, and the employer did not record it on their 2014 OSHA 300 log on or about April 11, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
