At this job site, personal fall arrest equipment was not in use; neither guardrails nor nets were installed.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On April 22, 2014, at this job site, an employee gained access to a residential roof under construction, using a portable ladder with the side rails "not" extended 3 feet above the landing surface. The employees were exposed to a fall hazard approximately 10 feet to the ground.
