29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) Wyoming, PA: Adequate safe means of access was not provided for workers performing overhand bricklaying from the platform of a fabricated frame scaffold that was approximately 13 feet above the point of access (ground level) exposing the workers to the potential for injury from a fall, on or about April 24, 2014. b) Wyoming, PA: Adequate safe means of access was not provided for a worker stocking brick from the platform of a fabricated frame scaffold that was approximately 16 feet above the point of access (ground level) exposing the worker to the potential from injury from a fall, on or about April 24, 2014. 29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system: a) Wyoming, PA: The open ends of the platform on a fabricated frame scaffold where workers were performing overhand bricklaying operations exposed workers to a fall of approximately 13 feet, on or about April 24, 2014. b) Wyoming, PA: A worker who was unloading brick from a forklift while standing on a single scaffold plank was exposed to a fall of approximately 16 feet, on or about April 24, 2014. ABATEMENT CERTIFICATION REQUIRED
