29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a) Worksite, 1149 Old Country Rd., Westbury, NY - An employee performing maintenance to an electric sign outside a commercial building was working from an Altec Model #AT35-G, Serial # 1207DE9063 bucket truck approximately 10 feet above the ground was not wearing a body belt or harness attached to the boom or basket; on or about 04/28/14.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
