29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Rolling Thunder, 7017 Roosevelt Boulevard, Philadelphia, PA- An employee performing re-roofing operations on a roof with a slope of approximately 4.5 inches in 12 inches (vertical to horizontal) was exposed to fall hazards of approximately 14 feet at the eave, 13 feet at the North gable end, and 27 feet at the South gable end, without any form of fall protection. Observed on or about April 24, 2014.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Rolling Thunder, 7017 Roosevelt Boulevard, Philadelphia, PA- An employee performing re-roofing operations on a steep barrel roof was exposed to a fall hazard of approximately 14 feet at the eave without receiving any fall protection training in the procedures to be followed to minimize the hazard. Noted on or about April 24, 2014.
