29 CFR 1910.23(d)(1)(ii): Stairways less than 44 inches wide having one side open, did not have at least one stair railing on the open side:    (a) Maintenance - The wooden stairway, with 17 steps, leading from the Maintenance Shop to a storage area did not have a complete stair railing on the open side (right side descending) in that the existing railing consisted of a top rail but lacked a midrail, exposing employees to the hazard of falling to a lower level, on or about April 24, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Maintenance - The underside of the table for the Marvel vertical tilt-frame band saw, used for cutting metal and plastic, had no guarding or enclosure on the left side, facing the machine, and only partial enclosures on the front and right sides, facing the machine, to protect the operator and other employees in the machine area from hazards such as the moving saw blade, on or about April 14, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one half the diameter of the shaft:    (a) High Volume Line - An unguarded horizontal shaft end, 3 feet 7.5 inches above the floor, on the tail end of the conveyor belt after the shear that cuts the fiberglass pads to size, had a diameter of 1 inch and projected 2.5 inches, exposing employees to a caught in rotating parts hazard, on or about April 24, 2014.    (b) High Volume Line Area - The Laminator for the High Volume Line had unguarded shaft ends that projected more than one half the diameter of the shafts, as follows:  four shaft ends each had a diameter of 1 inch and projected 1.75 inches to 2 inches ranging in height above the floor from 4 feet 1.5 inches to 6 feet; one shaft end had a diameter of 1.25 inches and projected 2.25 inches with a height above the floor of 4 feet 11.5 inches; two shaft ends each had a diameter of 1.5 inches and projected 2.25 inches with one of these shafts 4 feet 7.5 inches above the floor and the other five feet above the floor; exposing employees to a caught in rotating parts hazard, on or about April 24, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
